Job Description
Optum is hiring a Home Health Coder responsible for reviewing clinical documentation and assigning accurate ICD-10-CM diagnosis codes along with OASIS-based coding guidance. The role supports compliant and optimized reimbursement for home health services while ensuring coding accuracy and regulatory compliance.
The position involves close collaboration with clinicians and QA teams to resolve documentation gaps and improve coding quality.
Primary Responsibilities
- Review clinical documentation for Home Health services
- Apply accurate ICD-10-CM diagnosis codes
- Follow OASIS-based coding guidance
- Ensure timely and compliant claim submission
- Identify and resolve documentation gaps through provider queries
- Support PDGM (Patient-Driven Groupings Model) requirements
Skills/Qualifications
- Certified Medical Coder – AAPC / AHIMA (CPC, CCS, CPC-H, CCS-P, BCHH preferred)
- Graduate or Postgraduate in Life Sciences or Allied Medicine (BHMS, BAMS, BPT, Dental, Pharmacist, Nursing)
- Experience in Acute Surgery or Emergency Department (ED) coding
- Strong knowledge of Human Anatomy, Medical Terminology, and Diagnosis Coding
- Expertise in coding diabetes, wounds, fractures, CVA late effects, COPD, CHF, etc.
- Solid command over ICD-10-CM guidelines (Chapters 1-21) and etiology/manifestation rules
- Understanding of PDGM – Primary diagnosis selection and Comorbidity adjustments
- Excellent English communication and high analytical skills
- Ability to identify missing, conflicting, or insufficient documentation
- Willingness to learn and grow in a fast-paced environment
Key Responsibilities
- Assign accurate ICD-10-CM codes for Home Health episodes
- Apply OASIS guidelines for optimized reimbursement
- Perform PDGM-compliant primary diagnosis and comorbidity coding
- Identify documentation issues and issue provider queries when required
